Please feel free to contact us using the information below. We welcome the opportunity to meet you and to introduce you to our staff. In addition to all of the services we provide, we also have a $25 New Patient Offer that includes:
- Complete examination by the doctor
- Up to 2 (two) x-rays, if deemed necessary
- Review of the findings
Care in our office is usually covered by insurance, including auto insurance, Medicare and Medicaid (Hoosier Healthwise). We provide a complimentary benefits check for all of our patients. We will then let you know up front exactly what your coverage is and what out-of-pocket expenses you might incur before any treatment takes place. If you don’t have insurance coverage, we will work with you to find a way for you to still receive care.
